Client Bill of Rights
Please read and sign the following client bill of rights. I am happy to answer any questions you may have regarding your sessions.

My name is Morgan Oxley and I can be contacted by phone at 503 919 1418 and via email at morgan@morganoxley.com. All communications with me will be responded to promptly.
Education and Training:
I was trained in hypnotism at NW Hypnosis Institute. I am a Certified Member of the National Guild of Hypnotists and I do annual continuing education to maintain my training at a high level. The National Guild of Hypnotists is the oldest and largest hypnotism organization in the world and its certification is the most widely recognized credential for the professional practice of the hypnotic arts.

AS THE STATE OF Oregon HAS NOT ADOPTED EDUCATIONAL AND TRAINING STANDARDS FOR THE PRACTICE OF HYPNOTISM, THIS STATEMENT OF CREDENTIALS IS FOR INFORMATIONAL PURPOSES ONLY. Hypnotism is a self-regulating profession and its practitioners are not licensed by most state governments (including Oregon). I am not a physician nor a licensed health care provider and may not provide a medical diagnosis nor recommend discontinuance of medically prescribed treatments. If a client desires a diagnosis or any other type of treatment from a different practitioner, the client may seek such services at any time. In the event my services are terminated by a client, the client has a right to coordinated transfer of services to another practitioner. A client has a right to refuse hypnotism services at any time. A client has a right to be free of physical, verbal or sexual abuse. A client has a right to know the expected duration of sessions, and may assert any right without retaliation.
Redress:
I am a certified member of the National Guild of Hypnotists, and practice in accordance with its Code of Ethics and Standards. If you have a complaint about my services or behavior that I cannot resolve for you personally, you may contact the National Guild of Hypnotists at P.O. Box 308, Merrimack, NH 03054-0308, (603) 429-9438, to seek redress. Other services than my own may be available to you in the community. You may locate such providers in the telephone book.
Fees:
The charge for my services is $97 per session. Barter is accepted and I do offer discounts for financial disadvantage and pre-payment of multiple sessions. I reserve the right to change my prices without notice, however, the price you pay at your first session will be your price for at least one year from the date of your first session, should you wish to come and visit me for an unrelated issue or any other morganoxley.com reason. Credit cards, personal checks and cash are all acceptable forms of payment. Sorry, insurance payments are not accepted.
Insurance:
I suggest you think of my services as something that you will pay for personally. That will both protect your privacy and help you value the work you are doing more. In general, insurance companies do not like to cover hypnotic services, and I caution you not to expect them to do so.
Confidentiality:
I will not release any information to anyone without a written authorization from you, except as provided for by law. You have a right to be allowed access to my written record about you.
Minors:
Appointments for children under age 18 require written consent from the parent or guardian, who must accompany them at each visit.
Guarantee and Refund Policy:
No guarantees as to the effectiveness of hypnosis for your particular problem are made or implied, as it is impossible to guarantee human behavior or compliance. Therefore, no refunds for services are given. Hypnosis is not a quick fix or magic pill. A hypnotist is considered a guide or facilitator. You assume equal responsibility by making a commitment and allowing yourself to be guided into a state of hypnosis. No one can make you do something against your true will. I sincerely want you to succeed and pledge my efforts to help you to the best of my ability.
My Approach:
I believe that people can change. Just because you did something one way yesterday does not mean that you have to do it that way tomorrow. Using hypnotic technique and simple discussion I strive to enhance each client’s level of relaxation, playfulness and flexibility in daily life.
